Returning Student-Athletes

Update Student-Athlete Health Forms: 

Click the Returning User Sign-in and enter your username and password you created during your initial setup
From the menu, click Athlete Information and update the following information: 
Update/add any new information that has changed since your initial setup. (i.e. vaccination/immunization, home address, medical history etc.), be sure to click the Verify Athlete Information bar, located at the bottom of each tab page once you have updated/added any new information
Update & verify your current insurance information under the Insurance tab
Fill out and save the 4 medical forms under the Forms tab

How to update returning Student-Athlete Health Forms: 

From the menu, click Athlete Information, then find the Forms tab
Click on the Form Name bar, located at the top of the page and a list of the 4 forms will drop down
Click on one of the 4 forms, then push the NEW button and fill out the required information
Repeat for all required forms

Concussion Testing Returning Student-Athlete

Concussion baselines are required yearly. Gustavus contracts with SWAY Medical for our concussion pre & post testing. To access the app and complete your yearly baseline, please log in to your app store and download the SWAY Medical App. After you have downloaded the app proceed by using the team code below. If you are a multisport athlete, please use the code coordinated with the first sport you will participate in, for your baseline.

Incoming First-Year Student Athlete and Transfer Student Athletes

All items below must be completed in order for the Student-Athlete to be allowed participation on the first day of practice. First time users, please read through all instructions, watch the How-to Videos below and click link Athlete Profile to create your athlete profile.

Create your ATS Account: Click the link to begin your initial registration in ATS: https://www.atsusers.com/ATSWeb/login.aspx?ReturnUrl=%2fATSWeb%2fDefault.aspx%3fdb%3datsgustavus&db=atsgustavus

 

To create your profile use the following at the initial log in screen.
Athlete ID: new
Password: new

Once signed in, please fill out the highlighted fields to create your ATS profile 
Input your insurance information and emergency contacts under the Insurance & Emergency Contact Tab
Click Save Athlete Information at the bottom of your screen once all fields are completed
Once you have saved this information, new tabs will pop up on your screen (Medical History, eFiles, Screen/Test/Vaccine etc.) 
Input any previous surgeries under the Medical History tab

First year Student athletes fill out & save the 7 required forms located under the Forms tab, by clicking on the Form Name bar, then push the NEW button and fill out the required information; repeat for each form

All incoming athletes are required to complete a physical (first year and transfer)

Pre-Participation Physical Evaluation Form

Upload your completed physical form located under the eFiles tab - Health History & Physical Form
Under this tab you may download a copy of the Gustavus Pre-Participation Exam to take to you Physician to complete. All first year and transfer student athletes must have a physical within 6 months of the start of their season. The physical must be performed by an M.D., D.O., N.P., or PA. 

*SICKLE CELL TRAIT STATUS: Starting in 2022 the NCAA requires that all athletes know their sickle cell trait status. This is a test completed at birth for all newborns. Most newborn screenings in Minnesota may have been destroyed due to a lawsuit, check with your provider or obtain a new test. There is a line on the physical form to put the result of the test

Drug Testing Information 

Gustavus Adolphus College may test an athlete for banned substances. Below is a form that will be submitted if an athlete tests positive for ADHD medications at NCAA Sanctioned Events:
