In the NCAA’s ongoing efforts to support student-athletes beyond their playing days, the NCAA is pleased to announce that it has aligned with A-G Specialty Insurance to serve as claims manager of the NCAA Post-Eligibility Insurance Program, underwritten by Mutual of Omaha.

For up to two years (104 weeks) after student-athletes separate from school or voluntarily withdraw from athletics, the program covers excess accident medical expenses for athletically related injuries sustained during participation in an NCAA qualifying intercollegiate sport. The program provides excess insurance coverage for properly documented covered injuries that occur on or after Aug. 1, 2024.

This form will compile all the required information needed to file a claim.  The sports medicine staff will complete the claim form and upload all the required documentation from your previous injury sustained after Aug. 1 2024. The sports medicine staff will alert you of the completed claim and or if more information is needed.  Gustavus is not responsible for coverage of bills or services provided by medical providers or costs involved with the care of your previous injury.
